Summary: Fix VO installation; it was broken in 1.2.8.
Keywords: 

* src/org/glite/security/voms/database/DBCA.java (DBCA): Put the
  new instance in the instance hashes.
  (loadAllCAs): Remove instance hash handling.
  (create): Make sure the new CA is put in the instance hashes before
  trying to retrieve it from them.

* doc/NEWS: Describe changes.  (Describe some previous versions as well.)

* project/version.properties: Set version to 1.2.9.
